Hospice - RN Case ManagerA hospice RN case manager plays a crucial role in providing compassionate care to patients with terminal illnesses, coordinating their treatment plans, and supporting their families throughout the end-of-life journey.Responsibilities:Patient Assessment; Care Coordination; Direct Nursing Case; Education and Support; Documentation and Compliance; AdvocacyMust Haves:RN Registered Nurse in Texas or Compact state in good standing on license. A valid RN license is mandatory, along with certifications in hospice and palliative care being advantageous.Experience with RN Case Management in Hospice of at least one (1) year. Prior experience in nursing, particularly in hospice or palliative care settings is needed.Skills in crisis management and interdisciplinary teamwork are essential.Typically, a hospice RN case manager holds a Bachelor’s or Master’s Degree in Nursing, with relevant coursework in palliative care and healthcare management.
