Minimum of 1 year experience working with the frail or elderly population is required (no new grads)

Not accepting visa candidates

Provider must be interested in Perm (not accepting Locums)

ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.

Board Certified in Internal Medicine or Family Medicine - REQUIRED
Board Certified in Geriatric Medicine - PREFERRED
Active DE license - REQUIRED
CSR/CDS Certification (prior to start) - REQUIRED
DEA Registration - Verbal Confirmation (prior to start) - REQUIRED
Availability to begin working and state interest in Perm - REQUIRED
Minimum 1 year working with the frail or elderly population - REQUIRED
BLS (prior to start) - REQUIRED
Clean/Clear malpractice - HIGHLY PREFERRED
EMR TruChart experience - HIGHLY PREFERRED
Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
