JourneyCare Hospice in the Chicago area is seeking a compassionate Hospice RN Case Manager to provide patient-centered, end-of-life care. You will manage a dedicated hospice caseload and deliver skilled nursing care focused on comfort and dignity. Schedule is Monday–Friday, 8:30 AM – 5:00 PM with one weekend rotation every 8 weeks.
