Job Title: RN - Case Management Profession: Nursing Specialty: Case Management Duration: 13 Weeks Shift: Day Shift Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum 2 years of current acute care case management RN experience License: Active state licensure required Certifications: BLS Must-Have: Candidates must have: - Minimum 1 year of current acute care case management RN experience - RN or licensed social worker Description: The role involves case management assessment and discharge planning. Candidates without the required skills will not be considered. The work environment is very busy and fast-paced, requiring flexibility. The case management team is welcoming, collaborative, and supportive. The team is always willing to answer questions and assist new staff. The units have consistent physicians and mid-level providers to work closely with. Patients include those recovering from cardiac procedures and a variety of medical conditions. Post-acute placements are available, with many patients transitioning to cardiac rehabilitation.
