We are currently looking for a Part‑Time Medical Social Worker (MSW) to help serve patients in the Gallup area. Role Overview As a Hospice Social Worker, you will play a vital role in supporting patients and families through emotional, social, and financial transitions. Working closely with our interdisciplinary team, you will ensure each patient receives compassionate care that addresses their full spectrum of needs. Key Responsibilities Respond to referrals from the clinical team for social work support Perform in‑depth assessments to identify emotional, social, and economic concerns Offer brief counseling and assist families with navigating available resources Assist with planning, goal‑setting, and connection to community‑based services Keep accurate records and maintain documentation in accordance with care plans Participate in case discussions and contribute to interdisciplinary planning Provide home visits and be available for on‑call duties as required Maintain confidentiality while preserving patient dignity throughout care Required Qualifications Education: Master’s in Social Work (MSW) from a CSWE‑accredited program Experience: Minimum of one year in a social work role; hospice or healthcare experience preferred Licenses & Certifications: Licensed to practice Social Work in NewMexico Current CPR certification Valid driver’s license and reliable transportation with auto insurance Skills & Traits: Excellent communication and time management Deep empathy and a calm, professional demeanor Familiarity with Medicare/Medicaid and community resource networks Ability to work independently in home‑based environments What We Offer: Purpose‑driven work that truly impacts lives Flexible part‑time hours Supportive and collaborative work culture Minimal weekend and evening requirements (unless otherwise arranged) Ongoing training and career development opportunities Apply Today If you’re a compassionate, skilled social worker looking to support patients and families through one of life’s most meaningful stages, we welcome your application.
