About Duke University Hospital Pursue your passion for caring with Duke University Hospital in Durham, North Carolina, which is consistently ranked among the best in the United States. The largest of Duke Health's four hospitals with 1062 patient beds, it features comprehensive diagnostic and therapeutic facilities, including a regional emergency/trauma center, an endo‑surgery center, and more. Fellowship opportunity will start in February/March 2026. Must graduate as PNP‑AC by December 2025. Job Summary The Duke Pediatric Heart Center Advanced Practice Provider (NP or PA‑C) Fellowship rotates through clinical settings in the pediatric heart center, including inpatient and outpatient locations. The fellow will work alongside APPs and MDs to provide direct patient care services for newborn congenital heart, surgical, and medically ill pediatric heart center patients from birth to young adults. In addition to clinical care, the fellow contributes to excellence in patient care, research, teaching, and learns to provide leadership within the organization. Goals of Pediatric Heart Center Fellowship Acquire a deep and broad knowledge base of common congenital heart anomalies and disease. Understand the pathophysiology, various presenting clinical presentations, and disease prognosis for hospitalized patients. Develop a diverse differential diagnosis and treatment plan based upon the patient's presenting symptoms, using evidence‑based, cost‑effective medical care. Manage patients effectively throughout the hospital stay, including admission, daily rounds, continuation of evaluation, and dismissal, while remaining aware of the patient's or family's emotional, physical, and spiritual needs. Learn practice management, including strategies for training APPs and APP students. Develop keen verbal skills to communicate effectively with colleagues, physicians, patients, and ancillary staff. Job Duties and Responsibilities Clinical Expert In collaboration with cardiology attendings, PCICU attendings, surgeons, and/or APPs, round daily on patients to assess, diagnose, plan treatment, set priorities, and evaluate effectiveness and cost efficiency. Provide and coordinate clinical care for service‑specific patients through established protocols and under the supervising physician. Collaborate with residents, fellows, attending nurses, and patient resource managers to formulate treatment plans and monitor progress. Perform procedures appropriate to the environment after demonstrating competency. Assess patients for changes in status and institute appropriate interventions; initiate emergency interventions to stabilize patients until physician support arrives. Postoperative stabilization and management. Develop patient care pathways, protocols, and guidelines for specific problems in conjunction with physicians and team members. Analyze clinical practice patterns to improve care and increase effectiveness. Communicate verbally and in writing all pertinent patient information to supervising MD, nursing staff, patient resource manager, home health agency, and other providers. Document all relevant data in the patient’s medical record. Use prescriptive privileges only within scope of practice and credentialing protocols. Prescribe, order, and administer drugs, including schedule II–V drugs and necessary medical devices. Model competence in advanced nurse practitioner/physician assistant skills. Give daily handoff to appropriate APP, attending, resident, and/or intern at shift end. Assess educational needs of staff; provide one‑on‑one and formal education as needed. Provide culturally sensitive education to patients and families, using non‑technical language. Implement and evaluate educational programs for DUHS staff and other healthcare providers. Precept healthcare learners through contractual agreements. Research Utilization and Investigation Use current research and evidence‑based decision‑making in all clinical practice. Incorporate clinical research findings into standards of care. Support IRB‑approved clinical research/trials, including obtaining consent, ordering diagnostic tests, recording results, administering interventions, and reporting outcomes. Participate in interdisciplinary quality improvement and research activities. Measure and document outcomes. Consultant Provide expertise and information to other providers, patients, families, and the community regarding cardiothoracic diseases and procedures. Implement and evaluate educational programs for referring MDs. Develop and evaluate standards of care, policies, and protocols within specialization. Develop projects to advance Duke University Hospital and Duke University Health System goals. Professional Leadership and Development Model advanced practice professionalism. Participate in at least one professional organization. Maintain professional practice through active participation in the Cardiology department and professional organizations. Meet continuing education requirements and maintain professional certification. Enhance knowledge through publications, presentations, posters, and continuing education. Attend educational conferences, morbidity and mortality conferences, code reviews, NP monthly meetings, and adult congenital sessions twice a year. When caring for adult patients with congenital heart disease, consult and refer appropriately. Job Eligibility Requirements Education (must graduate by December 31, 2025) PA: Graduate of ARC‑accredited Physician Assistant Program; Masters or DMSc preferred. NP: Graduate of accredited MSN or DNP Pediatric Acute Care Nurse Practitioner Program. Certification PA: PA‑C NP: PNP‑AC License PA: Current unrestricted medical license issued by NCMB with authorization to practice as PA. NP: Current unrestricted RN license issued or recognized by NCBON with authorization to practice as NP. Both: Federal and State DEA with unrestricted full prescriptive authority. Experience New PA or PNP‑AC grads welcome to apply, no APP experience required. BLS (required) PALS (required) ACLS (recommended) Physical and Mental Requirements Able to articulate scope of practice within guidelines as a Nurse Practitioner / Physician Assistant. Able to independently seek out resources and collaborate on complex problems. Able to communicate clearly with patients, families, staff, and leadership. Able to use sensory and cognitive functions to prioritize information, perform assessments, treatment, and follow‑up. Able to use fine motor skills. Able to record activities, document assessments & interventions, prepare reports and presentations. Able to use computer and learn new software. Able to provide leadership in clinical area and meeting organizational goals. Able to navigate the hospital and DUHS to provide expertise to specific patient populations. Able to withstand prolonged standing and walking, and lift at least fifty pounds. Able to remain focused and organized. Able to work collaboratively with all levels of personnel. Job Hours Primary hours: minimum 12 to a maximum of 13; 12‑hour shifts every 4 weeks, covering 7 days a week (days, nights, weekends, holidays). Flexibility required to meet clinical needs.
