Hospice CNA – Homecare (Yadkin County)

Location: Yadkin County, NC
Schedule: Monday–Friday, 8:00 AM–5:00 PM, and every 6th weekend

As a Hospice CNA, you'll provide personal care and support to patients in their homes, assisted living communities, and skilled nursing facilities.

Responsibilities include:

• Assisting patients with bathing, dressing, grooming, toileting, feeding, and mobility
• Observing and reporting changes in patient condition to the nursing team
• Providing comfort, companionship, and emotional support to patients and families
• Helping maintain a safe and comfortable environment
• Documenting care provided and communicating effectively with the interdisciplinary team
• Working closely with nurses, social workers, chaplains, and other team members to support each patient's plan of care

• Current CNA certification
• Valid driver's license and reliable transportation
• Compassionate, patient-centered approach to care
• Strong communication and interpersonal skills
• Ability to work independently while remaining connected to a team

Hospice, home health, long-term care, and skilled nursing experience are helpful, but not required.
