Position Details

JOB OVERVIEW

The Certified Home Health Aide providing Attendant Care in Indiana, is responsible for providing all nonskilled ADL care as ordered by the physician, identified in the Plan of Care or PCA and that is within the scope of practice per state law under which includes but is not limited to the following

KEY RESPONSIBILITIES

Provide services that are ordered by the physician in the plan of care within the scope of practice per state law

Perform, assist, evaluate and document medical care, such as:Basic first aid

Read and record vital (temperature, pulse, respiration)

Range of motion exercises

Ambulation, transfer and positioning with wheelchairs and other non-life support medical equipment and devices

Prepare meals according to dietary and nutritional needs

Care for patient’s special conditions and needs within scope of authorized tasks

Help patient maintain good personal hygiene, such as:Assistance with bathing, toileting, grooming, shaving, skin and nail care, oral hygiene, dressing and eating

Assist with household management, such as:Home safety and sanitation

Infection control

General light housekeeping, laundry, and shopping specific to patient

Thorough completion and coordination of all required documentation, such asMaintaining and completing the necessary patient/family and Company documentation records upon conclusion of each visit

Observe and report significant changes in patient’s condition to the (RN) Nursing Supervisor

Adhere to confidentiality standards and professional boundaries at all times

Thoroughly complete and coordinate all required documentation

Observe and report significant changes in patient condition to the (RN) Nursing Supervisor

QUALIFICATIONS

Required

High school graduate or GED

Current CPR certification

Ongoing Education

Home Health Aide must receive at least 12 hours of in-service training during each 12- month period

In-service training may occur while an aide is furnishing care to a patient.

Demonstrated proficiency in clinical assessments, documentation and compliance with aide care and policies and procedures State Specific Requirements Additional state specific requirements

Additional state specific requirements:

Continuing Education as required by stateGeorgiaSuccessful completion of Nurse Aide training and competency evaluation program pursuant to the requirements of 42 CFR part 483, Subpart D; or

Health Care or Personal Care credentialing program recognized and approved by the state; or

Successful completion of 40 hour training program provided by a private home care provider which addresses at least the areas specified in the PHCP rules

IllinoisAppropriate clearing of Illinois Department of Public Health – Health Care Worker Registry,

Successful completion of Long-Term Care Assistants and Aides Training Programs Code; or

Documentation of current registration from another state; or

Successful completion of a Nursing Arts Program which included at least 40 hours of supervised clinical experience from an accredited nursing program as evidenced by diploma, certificate or other written verification from the school and proof of successful completion of the written portion of the Department established nursing assistant competency test; or

Successful completion of a US military Training Program that includes the content of the Basic Nursing Assistant Training Program as evidenced by a diploma, certification DD-214, or other written verification and successful completion of the written portion of the Department-established nursing assistant competency evaluation Proof of completion of a nursing program in a foreign country.

MassachusettsAdditional requirements for Complex Care Assistant Enhanced Care ServicesThis document serves to provide further clarity on the enhanced care tasks complex care assistants may perform in accordance with 130 CMR 438.415(C). The enhanced care services must not require the skills, judgement, or assessment of a nurse, and may be performed only after the complex care assistant is deemed competent in performing the task by the CSN agency or training organization. This document is subject to change.enteral G-tube/J-tube feedings – includes pump set up/discontinuation and/or administering bolus feeds; does not include changing or replacing of feeding tube.

skin care including application of OTC products or routine G-tube/J-tube care – application of non-medicated over-the-counter products or routine G-tube/J-tube care, or stomas requiring care (including cleaning, application of OTC products to enhance adherence of the appliance, application of the appliance) or simple dressing changes that do not require application of medications, medicated, or specialized dressing products.

oxygen therapy – provides assistance to replace oxygen tubing or nasal cannula and set oxygen at ordered flow rate so long as the care is not in response to a respiratory event requiring the judgement and assessment of a nurse.

oral (dental) suction to remove superficial oral secretions – provides suctioning of superficial secretions in the oral cavity, includes set up and cleaning of suction device.

ostomy and catheter care – empties/changes ostomy bag or urinary collection devices and cleans skin where there is no need for skilled skin care, recording, observation or reporting required. Does not include the replacement of catheters.

modified meal preparation – prepares diets that do not require nurse oversight to administer. This may include modification of meal consistency as directed.

equipment management and maintenance (wheelchair, CPAP/BiPAP, oxygen and Respiratory care equipment) and paperwork — simple cleaning, monitoring for and reporting any equipment issues to the RN supervisor and CSN agency, including associated agency paperwork.

braces, splints, and/or pressure stockings —application and removal of braces splints and pressure stockings.

transportation to medical providers / pharmacy (by driving the member or going alone).

incidental services – only when incidental to enhanced care service delivery; When a CCA provides services that meet the requirements of Complex Care Assistant, they may also provide some incidental services that do not meet the definition of CCA (for example, light cleaning after a member bathes) However, the purpose of the CCA service must not be to provide these incidental services, since they are not health related.

Identification and elimination of safety hazards

Practicing health protection and cleanliness by appropriate techniques of hand washing

Waste disposal and household tasks

Reminding the participant to self-administer medications

Provides assistance with correspondence and bill paying

Must be able to sufficiently reposition patients and move equipment without assistance

Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport

Driving

A valid driver's license and an acceptable Motor Vehicle Record (MVR) may be required.

Must be 18 years or older

Language

Must be able to speak, write, read, and understand English

Work Environment

Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions

Possible exposure to blood, bodily fluids and infectious diseases
