Case Manager

Company Description Centre Helps is a nonprofit helping agency based in Centre County, PA, dedicated to providing 24-hour emotional support, crisis intervention, and connections to critical community resources. The organization offers case management services for individuals and families struggling to meet basic needs, ensuring access to housing, food, and other essential supports. Centre Helps also delivers training and education in suicide intervention, empathic listening, local resources, and core counseling skills. In addition, the agency provides drug and alcohol harm reduction programming for young people. Role Description The Case Manager is a full-time, on-site role located in State College, PA, supporting individuals and families as they navigate crises and basic needs challenges. Day-to-day responsibilities include conducting client intake and assessments, developing service plans, and coordinating referrals to housing, food, mental health, and other social services. The Case Manager provides ongoing advocacy, follow-up, and emotional support to clients, while maintaining accurate documentation and case records in compliance with agency policies. This role collaborates closely with internal team members, local service providers, and community partners to ensure effective continuity of care. The Case Manager may also assist with community outreach, participate in trainings related to crisis intervention and harm reduction, and contribute to program improvement efforts. Qualifications Strong client service and case management skills, including assessment, service planning, and advocacy. Excellent communication and interpersonal abilities, with capacity for empathic listening and trauma-informed interaction. Organizational and administrative skills, including accurate documentation, time management, and attention to detail. Ability to collaborate effectively with multidisciplinary teams, community agencies, and diverse populations. Comfort working with individuals experiencing crisis, housing instability, mental health concerns, or substance use challenges. Basic proficiency with computers and case management or database systems. Bachelor’s degree in social work, psychology, human services, or a related field, or equivalent combination of education and relevant experience.
