Active West Virginia Physical Therapist license required prior to start. Home health experience required. Experience providing patient care in home settings preferred. Familiarity with HCHB EMR preferred. BLS certification from AHA or ARC may be required. Assignment Details Facility type: Home Health. Shift name: Days. Shift details: Monday–Friday, 0800–1600 (5x8). Weekly guaranteed hours: 40. Productivity expectation: 30 points per week. Coverage area: Kingwood, WV and surrounding two counties. Mileage reimbursed between visits from first patient to last patient.
