Job Summary and Qualifications Seeking a PRN Home Health Social Worker MSW to join St. David's Healthcare at Home, part of HCA Healthcare. Responsible for implementing standards of care for medical social work services, adhering to regulations, and establishing goals and priorities for social services to meet quality standards and support the organization’s mission. What you will do in this role Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment, and communicate findings to the registered nurse and interdisciplinary team. Provide assessment in the patient’s residence and assist when it is not safe, creating alternative plans as needed. Serve as a liaison between patients, families/caregivers, and community agencies, ensuring seamless home care services by providing direct oversight and coordination with other clinical providers. Participate in developing individualized care plans, involving the patient and family, and attend scheduled interdisciplinary group meetings to recognize psychosocial stresses affecting symptoms of the terminal illness. Qualifications Master’s Degree in Social Work or a related field (e.g., sociology, special education, gerontology, rehabilitation counseling, psychology). Previous experience in a healthcare setting working directly with individuals. Reliable transportation, valid automobile liability insurance, and a valid driver’s license.
