Responsibilities

• Implement and progress prescribed LTAC physical therapy plans of care under supervision of a licensed physical therapist.
• Lead and assist patient sessions focused on gait training, therapeutic exercises, balance, and activities of daily living.
• Monitor patient responses during treatment, document progress, and communicate changes to the supervising PT.
• Set up, apply, and monitor modalities and assistive devices per facility protocols; ensure equipment safety.
• Collaborate with rehabilitation and nursing teams to coordinate care and ensure patient safety and mobility goals.
• Provide patient and caregiver education on home exercise programs and safety techniques.

Requirements

• Active Physical Therapy Assistant license in Maine or eligible per state licensure requirements
• Associate degree in Physical Therapy Assisting
• Ability to work in a LTAC / skilled nursing setting
• Prior LTAC or SNF experience preferred but not required
• Strong communication and teamwork skills
