Reporting to the Clinical Coordinator, the ideal individual is flexible, compassionate, and professional. Location: 3864 N 27th Ave, Phoenix, AZ 85017 Schedule: Full-time, Employed Monday-Friday, 9am-6pm; No Weekends Additional Language Differential Pay Is Available Full Benefits, Including 401K, and Generous PTO/PST Position Summary: The Children’s Case Manager position is designed to provide effective case management, clinical perspective, and service coordination for families in accordance with the Arizona Practice Model 12 Principles. Under the Agency’s policies and professional requirements, the role participates as a member of an interdisciplinary clinical team. Managing clients by assisting with their treatment coordination, including but not limited to 6‑month and annual assessments, hospital admissions and discharges. The role also serves children who are involved with the Department of Child Safety and are not receiving other behavioral health services. The position completes intakes as required by site needs, assists teammates with case coordination for additional referrals, including referrals for high‑needs case management (HNCM), behavior coaching, out‑of‑home (OOH) packets, or referrals for psychological evaluations. The role communicates and documents client progress toward their family vision and treatment goals. This is a direct service position with opportunities to provide in‑office and in‑home family support and case management services. The role works under the direct supervision of the Children’s Clinical Manager. Duties Include: Conducts the engagement process with families to identify natural/formal supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA). Facilitates CFT/Service Plan meetings according to the Arizona Model and RBHA guidelines in order to create individualized service plans as identified by the CFT members. Ensures families understand the behavioral health process, the role of CFT Facilitator, and ensures family participation that links them to natural or community‑based supports. Coordinates and schedules CFT/Service Plan meetings with natural, formal supports, and system stakeholders. Submits appropriate referrals to clinical outpatient services within the agency and other behavioral health providers identified by the CFT. Updates and maintains electronic medical records on each case which include but not limited to Annual Assessment Update, RBHA demographics, Child and adolescent service intensity instrument (CASII), and CFT/Service Plan. Completes strength assessments and crisis/safety plans with families according to RBHA and the Arizona Department of Health Services (ADHS) guidelines. Develops positive supportive relationships with consumers, family, stakeholders, and other Terros employees. Completes Intake/Core Assessment as needed, according to RBHA and ADHS guidelines.
