Work Environment This position is an onsite role where you will be providing services directly to patients in their homes. Our facility emphasizes a supportive and resource-rich environment, designed to equip you with the tools and training necessary for optimal patient care. The scheduled hours for this position are Monday to Friday, from 9:00 AM to 5:00 PM, allowing for a balanced professional life. Key Responsibilities Conduct thorough assessments for each patient, gathering a detailed medical history and identifying specific rehabilitation needs. Develop customized treatment plans aimed at improving strength and mobility based on initial assessments. Provide one-on-one physical therapy sessions, helping up to 7 patients per shift, utilizing innovative therapy methods and techniques. Monitor and document patient progress accurately, ensuring all records are up to date and compliant with healthcare regulations. Communicate effectively with patients and their families, addressing any inquiries and keeping them informed about therapy milestones. Work collaboratively with other healthcare professionals, including occupational therapists and doctors, to ensure a holistic approach to patient recovery. Patient Population / Client Focus The patients at Moving & Grooving Rehab Center are primarily adults recovering from surgery, injury, or chronic illness requiring rehabilitation. We cater to individuals seeking to reclaim their daily functionality and improve their quality of life through effective therapeutic interventions. Your empathetic approach will play a crucial role in patient engagement and recovery. Qualifications An active Physical Therapist License issued by the New York State. A Bachelor's degree in Physical Therapy, with a Master’s or Doctorate preferred. Demonstrable knowledge of rehabilitation techniques and experience using therapeutic modalities. Strong interpersonal skills, with the ability to build rapport with patients and their families. Preferred Experience While a minimum of two years of hands-on experience in a similar setting is required, candidates with experience in home health care settings will be at an advantage. Familiarity with electronic health record (EHR) systems is also preferred, as it enhances documentation efficiency.
