Division Overview: The Community Resources and Treatment division (CRT) supports clients who may have struggles related to a substance use disorder. We provide opportunities for clients to rebuild their lives so they can become healthy and self-sufficient. CRT's work includes screening and assessments for substance use disorders, placing clients into treatment programs, monitoring and reporting on progress, and providing support, including but not limited to insurance enrollment, obtaining a state ID, and finding employment resources. We are currently looking for full-time - Care CoordinatorStarting at $44,676 - $47,000; contingent upon experience, education, etc. Position Summary: The Collaborative Bridges team is an innovative care coordination pilot program to connect individuals and families with substance abuse and mental health needs upon their discharge from hospitals and provide supportive care coordination services until linked to long-term community providers or client needs are resolved. This position will educate and guide patients on the critical role of primary care providers in maintaining their
overall health and well-being. Emphasize the benefits of having a dedicated healthcare professional who can
offer preventive care, manage chronic conditions, and coordinate specialized treatments when
necessary. Encourage patients to establish a relationship with a primary care provider within their
community to ensure continuity of care, timely medical interventions, and a trusted source for health
related guidance. Provide resources and support to help patients navigate the healthcare system,
select an appropriate provider, and understand the value of regular check-ups in promoting long-term
wellness. As part of a team, this position:
- Will help lead initiatives aimed at improving healthcare outcomes and reducing health disparities for justice-impacted populations.
- This role focuses on increasing access to preventive, primary, and specialty care within the community, addressing health-related social needs that impact access and overall health outcomes including chronic health conditions.
- Will work to enhance care integration, efficiency, and coordination across various provider types and care levels.
- Requires a strong commitment to health equity, strategic program development, and collaboration with healthcare and community partners to drive meaningful change.
- Partner with Collaborative Bridges staff to provide identified clients with comprehensive medical care in addition to any Medication Assisted Recovery or other evidence-based programs for their identified substance use disorder.
- Provide referrals/linkages to clients for identified needs and coordinate outreach services to community-based social service agencies that address social determinants of health - e.g., employment, housing, food security, transportation, etc.
- Ability to be respectful of the diverse cultures of the people served and to provide culturally appropriate, competent, and individualized treatment according to each client's age, gender, race, ethnicity, and culture
- Engage and Educate clients for participation in treatment and other outside needed resources for all Social Determinants of Health needs- e.g., employment, housing, food security, transportation, etc.
- Develop and maintain relationships with community-based programs and attend linkage appointments when necessary and appropriate
- Conduct outreach and follow up with clients, families, and treatment providers.
- Provide a Person-Centered approach to ensure client motivation in recovery.
- Assist with ensuring hospital discharge recommendations are understood by the consumer and followed through with by the patient.
- Ensure interventions are recovery-oriented, culturally congruent, and developmentally appropriate
- Engage natural and family support to strengthen the individual's participation and engagement.
- Complete intake packets for program enrollment and participation
- Provides referrals and crisis intervention for the individuals' identified needs
- Interview individuals, review records, and confer with other professionals to review individual progress and to determine their suitability for program participation
- Knowledge of DSM V and ASAM criteria
- Coordinate and implement the delivery of consistent and quality program services
- Maintain program information and technology affecting functional area(s) to increase program effectiveness and ensure compliance
- Prepare, interpret, and present reports to the Supervisor, external funding sources, and community partners in accordance with established timeframes
- Serves as direct liaison to the Supervisor for program-related needs and operational issues
- Assess, evaluate, facilitate, and monitor individuals' progress in relationship to established goals and objectives
- Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services.
- Identify and refer individuals with behavioral health needs
- Complete and maintain all required individual and program-related documentation, which includes documenting the results of screenings, enrollments, service plans, reports, progress notes, discharge summaries, and other individual-related activities, including accurate reporting of services
- Monitor all individuals determined by individual progress towards service goals
- High school diploma or GED equivalent
- Valid driver's license
- CADC certification within two years of employment with TASC may be required based on contractual obligations
- Must be able to pass an IDOC Background check
- Demonstrated proficiency in the Microsoft Office suite
- Demonstrated experience in group facilitation
- Knowledge of the current ASAM placement criteria, DSM-V as it relates to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire the knowledge
- Comfort working inside a correctional setting and with persons convicted of serious violent offenses
- Flexibility in thinking and approaches to leading group discussions
- Good verbal, written, and interpersonal communication skills
- Good time management skills with a proven ability to meet deadlines
