RN Case Manager

Job TitleUNIT DETAILS: Ability to assess, evaluate and drive discharge plan. Coordinate services to the patient's appropriate level of care. Ensure that treatment plan is initiated, evaluated and re-evaluated daily. Follow the progress and lack thereof daily. Monitor length of stay and identify barriers to discharge. Follow Medicare regulatory guidelines regarding patient choice, IMM notices and MOONS.
