RN - Case Management

This is a 13-week contract position.

Day Shift; 8:00 AM - 4:30 PM, with rotating weekends.

We are seeking a dedicated Registered Nurse (RN) for Case Management. In this role, you will provide comprehensive case management services for both inpatient and observation patients. Your responsibilities will include assessing psychosocial, medical, and discharge needs, coordinating care plans, and collaborating with a multidisciplinary team to evaluate care progression throughout the episode of care.

Responsibilities:

• Perform comprehensive assessments of patients and families.
• Coordinate discharge planning and care plans with the multidisciplinary team.
• Drive discharge plans and ensure timely progression of care using InterQual.
• Collaborate with Licensed Social Workers to address psychosocial needs.
• Provide education, counseling, and advocacy for patients and families.

Required Experience / Certifications / Licensure:

• Active RN License in Texas.
• A minimum of 2 years of acute case management experience.
