We offer a wide range of services including mental health care residential and outpatient substance use treatment and primary health services. Additionally we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health we empower individuals to overcome challenges by addressing social determinants of health fostering resilience and facilitating Resilient Recovery Program provides youth and adults with substance use and co-occurring mental health services in the form of individual and group counseling and care coordination. This program is committed to working with underserved communities from a variety of backgrounds. We strive to meet the ongoing needs of individuals struggling with mental health issues substance use disorders and co-occurring mental health Coordinators are an important part of the San Mateo County Outpatient Program. Utilizing a trauma informed approach they are responsible for assessing adult and youth client needs and strengths to collaboratively create treatment plans/problem lists with their clients that support their health and recovery. The care coordinator/counselor will provide individual and group counseling to adults and youth; deliver evidence-based curricula in accordance with program specific requirements. Care Coordinators assists clients in navigating systems of care while providing a supportive treatment environment. May work weekends and holidays as RESPONSIBILITIES Individual Treatment Responsibilities:Facilitates intakes/orientations comprehensive assessments individualized treatment planning crisis collateral case management and individual counseling sessions with clinical support and counseling to assist clients in meeting their treatment goals including providing feedback setting boundaries and celebrating links clients to both internal and external resources based on their treatment needs and follows up on the progress/ Setting Responsibilities:Facilitates group counseling sessions and teaches classes on various treatment-related screenings and intake assessments to determine clients eligibility and ASAM level of care crisis intervention and communicates with treatment team as unforeseen situations and attends required training and discharge planning in collaboration with the rest of the treatment with certifying agency requirements for registration/certification and facilitates client celebrations and special work one weekend day based on rotating schedule to facilitate the scheduled weekend work at agencys main office and in addition to co-location sites which may include schools community health centers in-home and partner Responsibilities:Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans/problem lists transition/discharge plans progress notes and appropriate updates in support of the health and recovery needs of the documents all individual and group counseling sessions intake and assessments completes discharge paperwork and referrals in a timely manner as required by DMC and accurate records by entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating participate in Peer Chart/ Utilization Reviews for preparation of program audits.
