Summary



The Public Health Nurse in the Pathways Clinic serves an important and integral role in supporting the Pathways team in providing medical care and care coordination to patients presenting for services, addressing concerns related to opioid, other substances use and urgent care needs. As part of an interdisciplinary team the PHN provides patient counseling and other related support to patients who use drugs and/or have a history of substance use disorder. The PHN provides oversight for a team of Medical Assistants, assures specimen collection and processing, assures follow up for Primary Care related concerns and assists in coordinating referrals and prescription access for Pathways patients.

Job Duties
  • Conducts nursing assessments and interventions, patient care planning, evaluation, and medical care coordination.
  • Carries out prescribed treatments, administers medications and vaccinations, monitors and documents treatment progress and patient response.
  • Conducts medical screening and triage with patients over the phone and in person.
  • Provide care management, assessment, education, and close follow-up for patients receiving who use drugs and/or have a history of substance use disorder.
  • Assist with ongoing wound care assessment, treatment, and management for established patients.
  • Provide point of care follow-up and preventive screenings for patients undergoing treatment for Opioid Use Disorder who use drugs and/or have a history of substance use disorder.
  • Lead quality assurance projects for clinic workflow and coordination of care with other healthcare and social welfare entities.
  • Provide education and preceptorship to BSN students and assist with education process for UW Addiction Medicine Fellows
  • Maintain partnerships with community outreach programs serving individuals living homeless and unstably housed needing referral and connection to behavioral health and primary care.
  • Other duties as assigned.

Experience, Qualifications, Knowledge, Skills

Requirements:
  • Bachelor's degree in nursing or a master's degree in nursing science or a graduate level nursing degree of MSN, DNP or PhD from an accredited school of nursing.
  • Washington State Registered Nurse License in good standing.
  • Washington State Drivers' License in good standing.
  • Current BLS-CPR for Healthcare Providers.
  • Successful completion of background check and credentialing
Work Schedule: This is a 1.0 FTE, fully benefited position and is overtime eligible. This is clinic-based nursing role and will be required to be onsite for direct services. Typical schedule is Monday-Friday 8 a.m. - 5 p.m. May work outside of the typical schedule to meet operational needs and will also support other clinics within the Downtown Public Health Center as needed.
