Case Manager RN

This is a 13-week contract position.

Day shift, 4x10-hour schedule (07:00 - 17:30). Weekend shifts required every other weekend or 4 weekend days in a 4-week schedule.

The Case Manager RN is responsible for discharge planning and care coordination in an acute care hospital setting. The ideal candidate will possess strong skills in managing patient needs, determining medical necessity per evidence-based guidelines, and performing continued stay reviews.

Conduct patient assessments and develop comprehensive care plans.

Coordinate with interdisciplinary teams to ensure quality patient care.

Manage and facilitate the discharge process for patients.

Review and validate insurance authorizations and continued stay criteria.

Provide education to patients and families regarding health management.

Ensure compliance with CMS regulations and HIPAA guidelines.

Current Alaska RN license required.

BLS certification preferred.

ACM certification preferred.

Minimum of 1 year of acute care experience is required.

Epic charting system experience is mandatory.
