Physician (MD/DO) Skilled Nursing Facilities Part Time

About the Role We're looking for an experienced, autonomous Physician to join our clinical team part-time, providing primary and episodic care toresidents in skilled nursing facilities. This is an independent role builtaround continuity you'll manage your own panel and build long-termrelationships with patients and their families. What's In It For You You build your ownschedule. Your Day-to-Day You'll see roughly 16–25 patients a day a mix of long-termchronic care and acute issues. That means annual wellness visits and H&Ps,managing chronic and co-morbid conditions, responding to acute changes,reviewing meds and orders, and coordinating with collaborating specialists andfacility staff to keep care seamless. You'll also stay in touch with familiesand join care conferences where it makes sense. Qualifications MD or DO from an accreditedmedical school. Board Certified (BC)required Internal Medicine, Family Medicine, or Geriatrics (or equivalent). Valid, unrestrictedphysician license in the state of operation. Successful completion ofan accredited residency program. Skilled nursing orlong-term care experience strongly preferred.
