Registered Nurse - Home Health

Job Description

Job Summary:

Provides comprehensive home health nursing care to patients in their homes, coordinating services based on physicians' orders and agency policies. Responsible for evaluating patient needs, implementing care plans, and supervising home health aides and licensed practical nurses.

Responsibilities:

• Coordinate and deliver patient care in a home setting according to established care plans and physician directives.
• Supervise and evaluate the performance of home health aides and LPNs/LVNs to ensure quality standards are maintained.
• Document patient assessments, interventions, and progress accurately using the HomeCare HomeBase (HCHB) EMR system.
• Travel up to 75 miles within the assigned territory to conduct visits, including start of care, wound care, and follow-up visits.
• Participate in periodic office meetings and maintain compliance with all clinical and documentation protocols.
• Adhere to safety, privacy, and infection control standards during all patient interactions.

Qualifications:

• Valid RN license in the state of practice, in hand with no pending restrictions.
• Minimum of 1 year of recent adult home health or hospice experience.
• Proficient in OASIS documentation and experienced with HomeCare HomeBase EMR within the last 6 months.
• Reliable transportation, a current driver's license, and agency-required liability insurance are essential.
• Able to travel up to 75 miles from the home clinic as needed.
• Strong communication and documentation skills, with the ability to manage a caseload of 5-8 patients daily.
