We are seeking compassionate, skilled, and dedicated Licensed Practical Nurses (LPNs) to join our team and help promote the health, safety, and well-being of the individuals we serve.

Location: Wethersfield, CT

Weekly Hours: 8-16 Hrs

Weekly Schedule: Saturday and Sunday, 7:00 AM - 3:00 PM, every other week.

What You'll Do

As a Licensed Practical Nurse, you'll support the health and well-being of individuals served by Oak Hill through a combination of clinical care, health monitoring, documentation, and direct support by:

• Providing clinical and nursing care
• Monitoring, assessing, and responding to the health needs of individuals served
• Following physician orders and nursing directives to ensure appropriate care and treatment
• Administering medications, injections, treatments, and other prescribed interventions
• Monitoring vital signs and health conditions, documenting observations accurately and timely
• Supporting specialized medical needs, including G-Tube care and other physician-directed health interventions
• Responding appropriately to medical emergencies and coordinating emergency care when needed
• Coordinating healthcare services
• Scheduling and coordinating medical appointments and follow-up care
• Communicating with physicians, pharmacies, hospitals, guardians, and interdisciplinary team members
• Maintaining medication records, nursing notes, medical consultation forms, and other required documentation
• Monitoring medication inventories, ordering supplies, and ensuring individuals have access to prescribed medications and treatments
• Promoting health, safety, and wellness
• Assessing health concerns and making recommendations regarding participation in daily activities, programs, and community outings
• Observing and reporting changes in physical, behavioral, or mental status
• Supporting preventive healthcare initiatives and wellness education
• Implementing health-related policies, procedures, and individualized care plans
• Supporting daily living and independence
• Providing guidance and assistance with personal care activities, including bathing, dressing, grooming, and hygiene when needed
• Encouraging independence and skill development while promoting dignity and self-esteem
• Assisting individuals with meal preparation, feeding support, and adherence to dietary guidelines
• Supporting participation in recreational, vocational, and community activities
• Maintaining documentation and regulatory compliance
• Completing nursing notes, incident reports, medical reviews, and other required records
• Participating in interdisciplinary team meetings and providing health-related updates
• Ensuring compliance with all Oak Hill, DDS, DPH, HIPAA, and regulatory requirements
• Maintaining confidentiality and accurate health records at all times
• Supporting a safe and inclusive environment
• Applying first aid, CPR, and emergency response procedures as necessary
• Utilizing approved lifting and transfer techniques, including Hoyer lifts and adaptive equipment
• Assisting with behavior support plans and other individualized programming as needed
• Safely transporting individuals to medical appointments, community activities, and other destinations using agency vehicles
• Collaborating as a team member
• Communicating effectively with colleagues, supervisors, nursing staff, families, and healthcare providers
• Participating in staff meetings, required trainings, certifications, and professional development activities
• Assisting with orienting and mentoring new employees as needed
• Contributing to a positive, mission-driven team environment focused on improving lives

What You'll Bring

Required Qualifications:

• Current Connecticut Licensed Practical Nurse (LPN) license in good standing
• Graduate of an accredited Practical Nursing program
• Valid driver's license with the ability to transport individuals in agency vehicles
• Current CPR and First Aid certification, or ability to obtain upon hire
• Strong clinical assessment, communication, and documentation skills
• Ability to pass required background checks, fingerprinting, and pre-employment health screenings
• Ability to complete and maintain required certifications, trainings, and regulatory requirements

Preferred Qualifications:

• Experience supporting individuals with intellectual and developmental disabilities
• Experience in residential, community-based, long-term care, behavioral health, or human services settings
• Experience with medication administration, G-Tube care, and health record documentation
• Familiarity with DDS regulations and person-centered support practices
