Inpatient Social Worker Case Manager

Kaiser Permanente seeks an Inpatient Social Worker Case Manager within our Healthcare & Medical Services department. In this role, you will provide psychosocial assessment, discharge planning, and care coordination for hospitalized patients and families. You will collaborate with physicians, nurses, and interdisciplinary teams to ensure safe transitions of care, connect patients to community resources, and address social determinants of health. You will also support crisis intervention, facilitate family meetings, advocate for patient rights, and maintain accurate documentation in our electronic health record to ensure high‑quality, patient‑centered care. Responsibilities Conduct comprehensive psychosocial assessments for hospitalized patients and families Develop and implement individualized discharge and transition-of-care plans Coordinate services with physicians, nurses, and interdisciplinary teams Connect patients with community, financial, and social support resources Provide crisis intervention, counseling, and safety planning as needed Advocate for patient rights and address barriers to care Facilitate family and care conferences to support shared decision‑making Document all interventions and plans in the electronic health record accurately and timely Assist with utilization management and appropriate level-of-care decisions Participate in quality improvement and care coordination initiatives Core Competencies Psychosocial assessment Inpatient discharge planning Care coordination Crisis intervention Motivational interviewing Knowledge of community resources Interdisciplinary team collaboration Clinical documentation in EHR Risk assessment and safety planning Utilization management basics Benefits Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Qualifications Current LCSW or LMSW (state licensure) BLS/CPR certification (preferred) Case management certification (e.g., ACM or CCM, preferred) Experience with hospital-based discharge planning Knowledge of community and social service resources Familiarity with EMR/EHR systems (e.g., Epic) #J-18808-Ljbffr Stryker Corporation
