UChange Home Care Services is hiring Certified Nursing Assistants (CNAs) to provide compassionate, reliable, and professional care to clients receiving Skilled Home Health Services in the 60419 and 60411 service areas.

Employment Type: Part-Time / Full-Time opportunities may be available
Schedule: Flexible scheduling based on client needs and availability

What You’ll Do

As a CNA providing home health services, you may be responsible for assisting clients with personal care and daily living activities while following the established care plan and agency policies.

Responsibilities may include:

• Assisting clients with bathing, grooming, dressing, and toileting
• Providing assistance with mobility, positioning, and transfers
• Assisting with activities of daily living
• Providing companionship and emotional support
• Assisting with meal preparation and light housekeeping related to the client's care
• Monitoring and reporting changes in the client's condition to the appropriate supervisor or healthcare professional
• Maintaining a clean, safe, and comfortable environment for the client
• Following the client's individualized care plan and assigned duties
• Maintaining accurate and timely documentation of services provided
• Using the agency's electronic systems for EVV, scheduling, and care documentation
• Communicating professionally with clients, families, supervisors, and other members of the care team
• Following all applicable agency, state, and client-specific policies and procedures
• Maintaining confidentiality and protecting client information

What We’re Looking For

We are seeking CNAs who are compassionate, dependable, professional, and committed to providing quality care.

Required or preferred qualifications may include:

• Current CNA certification in the State of Illinois
• Previous healthcare, home care, long-term care, or clinical experience preferred
• Strong communication and interpersonal skills
• Ability to work independently in a client's home
• Ability to follow a care plan and assigned instructions
• Reliable attendance and punctuality
• Reliable transportation when required for the assigned client
• Valid driver's license and automobile insurance when driving is required
• Reliable smartphone for communication, EVV, and documentation
• Ability to successfully complete required background checks and other applicable pre-employment requirements
• Authorization to work in the United States
