Health Center Lead, NP/PA

Quad Med Medical Clinic

The Health Center Lead, Provideris a patient-facing clinician responsible for delivering high-quality care while supporting the day-to-day operations of the health and wellness center.Approximately 90% of this role is dedicated to direct patient care. In addition to clinical responsibilities, this role provides on-site coordination and operational support to promote efficient workflows, team effectiveness, and a positive patient experience. The Health Center Lead partners closely withoperationsleadership and serves as a resource for staff, helping address routine operational needs and elevate issues asappropriate. Located onsite in New York City. Schedule 40hrs/wk

KEY RESPONSIBILITIES

Provides comprehensive primary care services, including assessment, diagnosis, treatment, and follow-up, in alignment with evidence-based guidelines. Performs physical examinations and preventive health services. Orders, interprets, and evaluates diagnostic tests. Prescribes medications and treatmentsin accordance withlicensure and clinical standards. Documents patient encounters, care plans, and outcomes accurately in the medical record. Educates patients and families on wellness, prevention, and treatment plans. Manages acute and chronic conditions,utilizingavailable tools and resources to support ongoing care. Collaborates with care team members to coordinate services, including referrals to internal and external providers. Maintains knowledge of pharmacological treatments, includingappropriate useand potential side effects. Adheres to all clinical protocols, regulatory requirements, and organizational policies. Works collaboratively with clinical and non-clinical staff to support a patient-centered care environment. Participates in quality improvement initiatives, patient care programs, and continuing education. Supports compliance with clinical, safety, infection control, and HIPAA standards. Serves as a clinical resource to team members for day-to-day questions and guidance. Supports daily health center operations, helping ensure efficient patient flow and resolution of routine issues. Partners with Operations Leadership tomaintaineffective workflows, staffing coordination, and service delivery. Assistswith staff scheduling updates and communicates coverage needs or gaps. Monitors operational processes such as work queues, logs, and supply levels, escalating concerns as needed. Helps address patient concerns and supports service recovery efforts. Communicates effectively with staff and leadershipregardingday-to-day operations. Participates in onboarding, training, and skill development of team members. Provides input tooperationsleadership on staff performance and operational improvements. Maintains EHR Super User knowledge and supports teamutilizationof systems. Responds to urgent or emergent situations, providing leadership and coordination as needed. Completes incident reporting and supports follow-up actions. Maintains confidentiality of patient and organizational information. Utilizes technology to communicate with patientsin accordance withorganizational policies. Demonstrates professionalism, adaptability, and effective communication in all interactions. Performs other duties as assigned.

JOB REQUIREMENTS

Education Completedmaster's degreelevel Nurse Practitioneror Physician Assistantprogram from an accredited college/university. Experience Minimumthree(3)yearsprimary care practice experience. 1 year of leadership experience preferred. Ability to work independently. Working with an Electronic Medical Record (EMR). Experience working with pediatric patients in some locations. Certain clientsand health centers mayrequiretravel. Certain clients and health centers mayrequirephlebotomy. Certificates, Licenses, Registrations Active and unrestricted applicable licensure. Current CPR (BLS)certification ormust be obtained within60 daysof hire.

NPI & DEA.

Current National Board Certification. May berequiredto obtain andmaintaina valid DOT Medical Examiner Certification, depending on job responsibilities or location. Ability to balance independent clinical practice with team collaboration. Effective verbal and written communication skills. Strong organizational and problem-solving abilities. Ability to support team coordination and daily operational needs. Proficiencywith electronic medical records and standard business software. Ability to manage multiple priorities and respond effectively under pressure.
