About the Role
We are seeking a Hospital Liaison to join our team of dedicated professionals. As a key member of our care coordination team, you will serve as a liaison between our center and healthcare providers, ensuring seamless transitions for our patients. This is a FULL_TIME opportunity, providing a chance for growth and development within our organization.
Key Responsibilities
Coordinate with healthcare providers to ensure accurate and timely transfer of patient information
Facilitate communication between healthcare providers, patients, and families to ensure continuity of care
Identify and address any issues or concerns related to patient transfers or care transitions
Collaborate with internal teams to ensure effective care planning and coordination
Demonstrate exceptional communication, interpersonal, and organizational skills
