As a Restorative Aide at Unity Point St. Luke's Living Center West, you will play a crucial role in assisting our residents in regaining and maintaining their independence through personalized care plans and therapeutic exercises. You will work closely with our interdisciplinary team to provide top-quality care to individuals in need of long-term care services.

• Assist residents with activities of daily living, including mobility, transferring, and range of motion exercises
• Implement and monitor individualized care plans to promote optimal physical functioning and independence
• Collaborate with nursing staff and therapists to ensure continuity of care and progress tracking
• Document observations and progress notes accurately and in a timely manner
• Provide emotional support and encouragement to residents throughout their rehabilitation journey

• Certified Nursing Assistant (CNA) required
• Prior experience working in a long-term care setting
• Strong communication and interpersonal skills
• Ability to work effectively as part of a team
• Compassionate and patient demeanor
