Physical Therapist Floater - Full-Time (Home Care & Outpatient) $48.00-$52.00 per hour Full-Time | Monday-Friday | 40 Hours per Week | No Weekends Join Community Physical Therapy's Continuum of Care Program, where you'll have the unique opportunity to provide both home care and outpatient physical therapy services within Alden's Assisted Living and Independent Living communities. This role allows you to build lasting relationships with your patients by following them throughout their rehabilitation journey, delivering seamless, high-quality care in both the home and outpatient settings. What You'll Do Treat residents within Alden Assisted Living and Independent Living communities Deliver individualized, patient-centered care throughout the continuum Collaborate with an experienced interdisciplinary therapy team Document: Home care services in Homecare Homebase Outpatient services in TheraOffice Why You'll Love This Role $48.00-$52.00 per hour Monday-Friday schedule No weekends Flexible scheduling between 7:00 a.m.-6:00 p.m. Variety of home care and outpatient patients Follow your patients from evaluation through discharge for true continuity of care Therapist-led, patient-centered organization Supportive leadership and collaborative team environment "To work together as a team, providing and stimulating improvement of the whole while fostering individuality and professional growth."
