About the Position

Job Title: Home Health Registered Nurse Profession: Nursing Specialty: Home Health RN Duration: 13 weeks Shift: Tuesday - Saturday or Sunday - Thursday Hours per Shift: 8am to 4pm Experience: Minimum of 2 years License: Valid nursing license Certifications: OASIS experience required Must-Have: - OASIS experience - Ability to assess patient and caregiver needs Description: Utilizes the nursing process to assess patient and caregiver needs in order to provide quality care to assigned patients in accordance with evidence-based practice and regulations. Develops and communicates a realistic plan of care for each patient in accordance with evidence-based practices and regulations. Serves as Case Manager for assigned patients and actively manages caseloads and visit times. Effectively completes OASIS documentation in accordance with policies and procedures, ensuring timeliness and compliance with requirements. Implements medical plans of care, nursing interventions, and procedures as necessary to ensure safe and effective quality of care. Maintains up-to-date knowledge of various high-tech skills such as IV use and maintenance, wound VAC, and complex wound care. Collaborates with LPNs to ensure that the plan of care is followed, delegates tasks, and ensures patient needs are met. Evaluates different aspects of the implemented plan of care to meet patient and caregiver needs. Maintains accurate documentation of nursing care provided to ensure effective integration of information for the health care team. Follows agency protocol for documentation and patient interaction processes. Performs patient and caregiver education to ensure informed decision-making regarding care. Identifies interdisciplinary needs of the patient and documents these needs accordingly. Completes various forms in a timely manner and ensures availability during visits. Manages visits in a time-effective manner and communicates with scheduling staff as needed. Assesses patients' needs for supportive services and coordinates care with patients and caregivers. Submits patient incident reports promptly when necessary. Coordinates safe patient discharges and transfers, ensuring appropriate paperwork is processed. Collaborates with medical professionals, providers, team members, and vendors to support patient care. participates in peer review audits and provides constructive feedback. Schedules and facilitates case conferences to maximize team communication. Advocates for patient care issues to ensure quality care is sensitive to patient and family needs.
