Position Overview (HIGH PRIORITY – SLC ROLE)
We are seeking a dynamic, experienced Supported Living Coach (SLC) to support individuals with developmental disabilities in navigating independent living, community resources, and complex life needs.
This role goes beyond caregiving—you will act as a coach, advocate, and guide for individuals managing housing, entitlements, and life skills.
What You'll Do
Provide Supported Living Coaching (SLC) services in accordance with APD guidelines
Assist individuals with independent living skills (budgeting, cooking, scheduling, hygiene)
Navigate and support housing resources:
HUD programs
Section 8
Subsidized housing applications
Support with entitlements & benefits:
SSI / SSDI
Medicaid / Medicare
Food Assistance (SNAP)
Coordinate with APD Support Coordinators, families, and care teams
Promote community integration (appointments, activities, employment readiness)
Maintain accurate documentation (Therap / iConnect)
Ensure compliance with AHCA, APD, and Medicaid waiver requirements
Requirements (MUST HAVE)
Minimum 2+ years of experience working with individuals with developmental disabilities
Experience as a Supported Living Coach (SLC) or similar role (SLC certification by APD)
Strong knowledge of:
APD Waiver services
Medicaid systems
Community resources and referrals
Experience navigating:
Housing (HUD, Section 8, low-income programs)
Government benefits (SSI, SSDI, SNAP)
Strong documentation and organizational skills
Reliable transportation
High school diploma or equivalent (Bachelor's preferred in Human Services or related field)
