GILEAD Case Manager- Union

Gilead Community Services is seeking a caring and recovery-oriented Clinical Case Manager to join our multi-disciplinary residential program staff. About the position:The Clinical Case Manager for Gileads Old Saybrook area adolescent residential program works Sunday & Monday 3:00 PM - 11:00 PM Friday & Saturday 3:00 PM - 11:00 PM (32 hours per week) providing case management advocacy educational and supportive services in a program serving adolescents with major mental illness and co-occurring substance use disorders. The individual in this role carries out all phases of clinical services including intake engagement assessment treatment planning case management referral and discharge. These standards are outlined in Gileads Standards of Conduct policy.
