Home Health Physical Therapist West Portland, OR & Surrounding Areas
Join a supportive Home Health team serving the West Portland area and surrounding communities! We are seeking an experienced Home Health Physical Therapist for a full-time day shift position.
Position Details:
- Day shift | 5x8-hour (8:00 AM 4:30 PM)
- Up to 6 patient visits/day | Standard 30 productivity points
- West Portland & surrounding areas
- IRS Standard Rate mileage reimbursement
- Charting: EPIC Remote Client (laptop) + Rover app
- ASAP (local candidates accepted)
Requirements:
- Minimum 1 year of skilled Home Health PT experience
- Active PT license in Oregon
- OASIS experience required
- Reliable transportation
- Ability to travel within an occasional maximum radius of ~60 miles (6090 minutes total drive time/day)
Typical Patient Diagnoses: Dementia, Parkinson's, ALS, MS, Ortho Post-Op, Post-Stroke, and other medically complex conditions.
