Growing together. Positions in this function perform advanced Patient Care Coordinator duties with a high level of autonomy and competency. Serves as a key member of the medical team, delivering excellent customer service and supporting strong patient-clinician relationships. In addition to core responsibilities, handles administrative tasks, which vary by department. Schedule: Monday-Friday, 8:30am-5pm EST. Schedule may vary depending on business need. Location: 1177 Boston Providence Turnpike, Norwood, MA 02062. Department: Internal & Family Medicine Primary Responsibilities:
- Scheduling, rescheduling, and confirming patient appointments
- Checking patients in and out and verifying demographic information
- Answering phones and directing calls to the appropriate staff
- Responding to patient questions and providing general information
- Processing referrals, authorizations, and medical records requests
- Coordinating communication between patients, providers, pharmacies, and other departments
- Sending and receiving medical records and documentation
- Maintaining accurate and confidential patient information in the electronic medical record (Epic)
- Assisting with prescription refill requests according to office workflow
- Preparing forms, letters, and other patient/provider documentation
- Supporting office workflows and maintaining efficient patient flow
- Following HIPAA, privacy, and organizational policies
- Assisting providers and clinical staff with administrative needs
- Participating in team meetings, training, and process-improvement initiatives
- Providing excellent customer service and helping create a positive patient experience

- High school diploma/GED
- 1+ years of customer service or medical office/healthcare experience
- Access to reliable transportation
- Associate's Degree in Medical Office Administration Program
- Proficiency in Microsoft Office
- Proficiency with scheduling software and electronic medical records systems (Epic or equivalent)
- Proven solid problem solving and complex patient management skills
