Pathway of Hope Case Manager

Position Description Position Title: Pathway of Hope Case Manager Location/Dept: Midwest Corps Reports To: Corps Officer FLSA Category: Non-Exempt Status Type: RFT Pay Grade: 205 Pay Range: $21.81 – $26.17 The Pathway of Hope Case Manager is responsible for managing Pathway of Hope casework services to families at the Midwest Corps. The Case Manager works under the guidance of the Regional Social Work Manager and the Corps Officer to implement the program’s objectives: providing services to Pathway of Hope families, keeping required records, and identifying resources to support participating families. Essential Duties and Responsibilities Provide casework services to ensure full implementation of the Pathway of Hope model within the assigned Corps. Program Implementation Provide intake and assessment services as defined by the Pathway of Hope curriculum. Deliver the full range of Pathway of Hope individualized services to a minimum of 20 families annually, including: Conduct pre‑screening and intake of participants using specified assessment tools. Confirm eligibility for low‑income guidelines with emphasis on vulnerable populations. Provide linkage to Pastoral Care Representative for pastoral care, counseling, and support. Conduct goal-setting steps with participants to formulate change‑oriented action plans. Hold follow‑up case‑management meetings with participants. Refer participants to internal and/or external services to address identified goals. Monitor and track participant progress toward goal attainment on action plans. Develop plans to address transition needs and maintain ongoing documentation of progress. Enter client data and reporting information into the SIMS system to support local, national, and territorial POH program outcomes. Maintain case files for each client that include all assessments, action plans, case notes, and graduation plans. Participate in POH planning and program development at assigned sites. Attend scheduled cluster meetings, conference calls, and individual consultation/supervision meetings with the Regional Social Work Manager. Attend meetings with the Corps Officer and other Corps staff. Attend divisional or territorial educational forums and webinars to facilitate understanding of a wide range of topics, especially related to case management and POH. Maintain working relationships with community agencies to provide comprehensive services and stay abreast of current trends and resources. Conduct outreach initiatives during pantry distribution times, within the community, at community meetings, etc. Develop and participate in outreach and community partnership initiatives to strengthen community engagement and access to services. Pathway of Hope Program Evaluation and Outcomes Measurements Ensure the accuracy of data entry into the database. Support the POH implementation plan as requested or assigned. Serve on committees related to case management, emergency assistance, and building sufficiency, as requested or appointed. Program Evaluation and Certification Maintain records in compliance with standards to assure program evaluation/certification requirements. Other Duties: As assigned. Reporting Relationships This position reports to the Corps Officer(s) with guidance from the Regional Social Work Manager. It will work in conjunction with the Program Services Analyst and the Regional Social Work Manager to support reporting and data collection needs, when necessary. The position works collaboratively with Corps Officer(s) and social services to ensure effective delivery of quality services. In contact related to position duties, the individual acts as a representative of The Salvation Army. Performance Measurements The individual will be evaluated on effective achievement of outcomes, as well as timeliness, accuracy, and completeness of assigned goals. A 30‑, 60‑, and 90‑day performance review will be conducted. Education & Experience Bachelor’s degree in a human service area required; BSW preferred. Minimum of two years of casework or social service experience. Valid driver’s license and clearance of The Salvation Army’s MVR process. Bilingual (English/Spanish) with ability to communicate fluently in Spanish preferred. Complete the Territorial Caseworker Certification Program within the first 90 days of hire. Capability to work with confidential material. Position Limitations Commit only to Army resources that have been allocated or approved. Keep the Regional Social Work Manager and the Corps Officer(s) informed on all critical issues. Adhere to all policies and procedures. Physical Demands / Work Environment Moderate physical work with ability to lift 25 lbs. Regular use of standard office equipment and PC. Work environment – office with low noise level. Travel to assigned service sites. Maintain positive workplace atmosphere through respectful and inclusive communication and actions.
