Physical Therapist

Role Overview: A licensed Physical Therapist provides personalized, one-on-one care to older adults in private homes and senior living communities, focusing on improving mobility, independence, and quality of life. The role emphasizes autonomy, flexibility, and building strong patient relationships, with opportunities to incorporate telehealth and virtual care for enhanced access and continuity. Key Responsibilities Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy Support patient wellness, mobility, and functional independence through therapy sessions Document care effectively within EMR systems in compliance with regulations Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists Educate patients, caregivers, and staff on safety and aging-in-place strategies Utilize telehealth when appropriate to extend care access Qualifications & Skills Valid Physical Therapy license or eligibility in the state(s) of practice Degree from an accredited Physical Therapy program Proficiency with basic computer and EMR systems Current CPR certification Physical & Practical Requirements Ability to perform moderate physical effort, assist with patient transfers, navigate stairs, and drive between locations. Must be able to wear PPE and provide assistance as needed.
