Job Summary:

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our dedicated team at Ditmas Park Nursing & Rehab. As a Licensed Practical Nurse, you will provide basic medical attention to patients and assess their wellbeing as part of a larger medical team. Your duties will include checking vital signs, recording a patient's medical history, and assisting patients with hygiene.

Responsibilities:

• Administer medications and nursing procedures to patients as prescribed.

• Obtain and record patient temperatures, blood pressures, pulse rates, and other vital signs using standard instruments and procedures.

• Observe, monitor, and record patient symptoms and conditions to obtain and provide information for diagnosis and treatment; identify responses to medication or changes in health status, and reports status to a registered nurse, physician, or other licensed healthcare provider.

• Prepare patient for or assist with examinations and treatments.

• Maintain charts and other records of patient conditions and/or administered treatment.

• Educate and counsel patients regarding their health, medications, and treatment goals and procedures, as appropriate.

• Monitor supply inventories, and complete requisition forms to ensure adequate supply levels.

• As necessary, observe and supervise the activities of unlicensed direct care staff, including the administration of medication, to ensure compliance with applicable requirements and report observations to higher-level staff.

Education / Certification:

• Currently registered in New York State or has possession of a valid permit to practice as a Licensed Practical Nurse.

Experience:

• Previous long-term experience in LPN role is preferable.

• Good investigative skills.

• Ability to write, educate, and work independently.

Job Type: Full-time

Schedule: 8 hour shift, Monday to Friday and every other weekend.

Work Location: One location, Brooklyn, New York.
