Clinica Recovery LLC is in search of a dedicated and licensed Homecare Physical Therapist (PT) to join our dynamic team in East Brunswick, New Jersey. Work Environment

This position is primarily an onsite role conducted in the homes of our patients throughout East Brunswick and surrounding areas. You will operate from our outpatient office, which serves as a hub for coordination and support, but your day-to-day interactions will involve traveling to patients' residences. Our office environment is supportive and collaborative, encouraging ongoing communication among team members to ensure continuity of care.

Key Responsibilities

• Conduct thorough in-home assessments to evaluate patients' physical conditions, which includes muscle strength, coordination, and mobility.
• Design and implement personalized physical therapy treatment plans that are evidence-based and tailored to the specific needs of each patient.
• Utilize advanced therapeutic techniques such as manual therapy, orthopedic rehabilitation, and neuromuscular re-education during therapy sessions.
• Provide guidance on therapeutic exercises, gait training, and balance retraining, ensuring that patients understand proper techniques and precautions.
• Educate patients and caregivers on home exercise programs, self-management strategies, and the overall treatment process to empower them in their recovery journey.
• Maintain clear and timely documentation of patient progress, treatment adjustments, and compliance with Medicare and agency standards, employing our robust electronic health record system.
• Collaborate with a multidisciplinary team including physicians, nurses, and occupational therapists to ensure a comprehensive approach to each patient's care plan.

Patient Population / Client Focus

Your work will primarily focus on diverse groups of patients, ranging from post-operative recovery clients to those managing chronic conditions or disabilities. This role demands a compassionate approach, as many patients may feel vulnerable in their home environments. Building rapport and trust is essential to fostering effective therapy and motivating patients on their paths to recovery.

Qualifications

• A valid New Jersey Physical Therapist license is required.
• A Bachelor’s degree in Physical Therapy from an accredited program (a Doctor of Physical Therapy or equivalent is preferred).
• Strong clinical skills with a minimum of 2 years of experience as a licensed Physical Therapist.
• Excellent communication and interpersonal skills to effectively engage with patients and their families.
• Ability to work autonomously in a home care environment while efficiently managing your own schedule.
• Valid driver’s license and access to reliable transportation for patient visits within the designated territory.

Preferred Experience

• Home health experience is highly desirable but not mandatory.
• Background in working with geriatric populations or patients with complex medical conditions is a plus.

Compensation and Benefits

We offer a competitive salary ranging from $110,000 to $125,000 per year based on experience, along with comprehensive benefits that include:
