We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to provide high-quality patient care in a fast-paced healthcare environment. Overview:
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to provide high-quality patient care in a fast-paced healthcare environment. The LVN will work closely with providers, nurses, and multidisciplinary teams to support patient health through clinical care, education, and care coordination while maintaining the highest standards of professionalism and patient confidentiality. Key Responsibilities:
- Deliver patient care in accordance with established nursing standards, policies, and procedures.
- Process reportable communicable disease reports as required by local and state regulations.
- Assist with prior authorizations and medication management.
- Review laboratory results and communicate abnormal findings to providers.
- Triage patient phone calls, voicemail messages, and electronic patient portal communications.
- Administer medications, collect blood samples, and initiate IV access (if certified).
- Support and supervise Medical Assistants under the direction of the Nurse Manager.
- Help ensure efficient clinic workflow and timely patient care.
- Coordinate patient outreach and follow-up appointments as needed.
- Perform patient assessments and implement nursing care plans.
- Provide compassionate emotional, psychological, and educational support to patients and their families.
- Educate patients regarding diagnoses, medications, treatment plans, and self-care.
- Maintain accurate and timely clinical documentation in electronic medical records.
- Ensure compliance with infection control, OSHA, HIPAA, and other applicable healthcare regulations.
- Monitor inventory of medical supplies and equipment.
- Participate in staff meetings, case conferences, training sessions, and continuing education activities.
- Maintain patient confidentiality and deliver culturally sensitive, non-discriminatory care.
- Perform other duties as assigned.
- Valid Licensed Vocational Nurse (LVN) license in the State of California.
- Current Basic Life Support (BLS) certification.
- Ability to provide proof of required immunizations upon hire.
- Minimum of two years of experience providing direct patient care as an LVN.
- Experience in HIV/AIDS, Hepatitis C, chronic disease management, or infectious disease care is preferred.
- Knowledge of communicable disease reporting requirements is preferred.
- Strong understanding of psychosocial, behavioral, and trauma-informed patient care.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively with diverse patient populations in a multicultural healthcare setting.
- Additional clinical certifications are considered an asset.
- Ability to successfully complete pre-employment screening, including drug testing, physical examination, background verification, and employment history checks.
- Strong attention to detail and organizational skills.
- Ability to work flexible schedules as needed.
- Compliance with annual health screening and immunization requirements for healthcare personnel.
