About the Position

Job Title: Case Manager - RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 5x8 (08:00 - 14:30) Experience: Minimum of 4 years required. No first-time travelers. License: Licensed to practice as a Registered Nurse in the state of North Carolina. Certifications: BLS Required; ACM/CCM Preferred Must-Have: - Two years of healthcare experience as a Registered Nurse. - EPIC experience required. - Acute hospital experience required. - Must be within a 50-mile radius for travel positions. Description: The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction. This position promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Case Manager must be a highly organized professional with great attention to detail. The individual must be adaptable to frequent change and compliant with regulatory and departmental guidelines and policies.
