Responsibilities

• Conduct comprehensive speech, language, cognitive-communication, and swallowing assessments for adult residents
• Develop, implement, and monitor individualized treatment plans; reassess progress regularly
• Provide direct speech-language therapy to residents in a skilled nursing setting; adjust plans based on clinical progress and goals
• Document evaluations, progress notes, treatment plans, and discharge plans in the facility’s electronic medical record
• Collaborate with the interdisciplinary team (nursing, OT/PT, physicians, social work) to support resident outcomes and care planning
• Support caregiver and family education; participate in care conferences and discharge planning as appropriate

Requirements

• Master's degree in Speech-Language Pathology from an accredited program
• Active state licensure to practice Speech-Language Pathology in New Mexico or eligibility for licensure
• Experience with adult populations; long-term care or skilled nursing setting experience preferred
• Ability to perform clinical assessments, create treatment plans, and document care accurately in an EMR
• Strong communication and collaboration skills; comfortable working in a team-based environment
• Willingness to participate in a travel assignment and adhere to the shift and schedule requirements
