Hospice Chaplain

JOB PURPOSE: To plan and coordinate spiritual services for PruittHealth Hospice families and patients. KEY RESPONSIBILITIES:
- Ability to interact with a variety of different age groups.
- Ability to work well independently and within an interdisciplinary team setting.
- Ability to communicate effectively orally and in writing with individuals and groups.
- Ability to handle confidential matters.
- Ability to work a flexible schedule.
- Possess knowledge of computer software, including proficiency in word-processing.
- Provide spiritual services to patients and families based upon their identified need (spiritual values and beliefs are NOT to be imposed upon the patient/family).
- Assess the spiritual needs/concerns of patients and families through direct communication with the individual, the patients clergy, or by information obtained by other Hospice personnel.
- Ensure that spiritual support is available to patients and families in keeping with their wishes and desires.
- Provide consultation education and support for other members of the hospice team, healthcare centers and facilities, and the community.
- Performs sacerdotal functions as necessary to palliate the spiritual crises of patients and families.
- Assist with volunteer training as requested.
- Serve as a member of the Interdisciplinary Group to assist in identifying and addressing spiritual needs and developing care plans, including attendance at team conferences for residents of healthcare centers and nursing facilities.
- Assist with on-call chaplaincy coverage.
- Act as a liaison to community clergy.
- Provide grief and bereavement counseling to families.
- Conduct memorial/funeral services as requested.
- Perform other duties as assigned.
- Keep supervisor informed of daily activities and issues.
- Must get along with coworkers, maintain effective relationships with management staff, employees, and the general public.
- Follows all company policies and procedures, state and federal laws and regulations and charged with reporting violations to the appropriate supervisor.
- Carries out all duties with a Commitment to Caring and in accordance with the companys mission, vision, and philosophy.
- Maintains confidentiality of company information.
- Demonstrates honesty and integrity at all times.
- Responds to change productively and handle additional projects as assigned.
- Able to carry out the duties and responsibilities of this job (with or without reasonable accommodation) without posing specific or current risk of substantial harm to the health and safety of self or others.
- Will coordinate patient care responsibilities with Director of Health Services and other members of the interdisciplinary team.
- Perform other duties as assigned by Administrator.
