Home Health Physical Therapist

Well Care Home Health of the Triangle, Inc. seeks a Home Health Physical Therapist to provide one-on-one, in-home care to patients across the Triangle. In this role, you will complete comprehensive PT evaluations, develop individualized plans of care, and deliver evidence-based interventions focused on safety, mobility, and functional independence. You will collaborate closely with nursing and other therapists, educate patients and caregivers, and document using EMR systems.
