Requirements:

• Pharmacy degree from a fifth-year college or university program equivalent
• Indiana Pharmacy License
• Preferred: Recent experience as a hospital pharmacist within the last 5 years. New graduates with robust hospital work histories will be considered.
• Preferred: PharmD and/or PGY-1 Residency
• Will accept retail pharmacist experience
• Education: Minimum Level of Education Required: Doctorate degree
• Preferred educational qualifications: Pharmacy Residency
