Comfort Keepers is seeking a highly organized, compassionate and proactive Client Care Coordinator (CCC) to join our Oklahoma team. This is a key operating role responsible for managing client relationships and helping ensure seniors and veterans receive reliable, high-quality care in their homes.

The CCC serves as a primary point of contact for a portfolio of clients and families, working closely with caregivers, our scheduling team, General Manager and nursing team. You’ll be involved throughout the client journey—from responding to prospective clients and conducting care consultations to helping launch new cases, coordinating caregivers, resolving service issues and maintaining strong relationships with existing clients.

This is an excellent opportunity for someone who enjoys people, problem-solving and getting things done . It is not strictly a desk job: you’ll spend time in our Oklahoma City office as well out as visiting clients, caregivers, healthcare partners and prospective clients in the community.

What You’ll Do

· Respond to new client inquiries, handle intake calls, follow up with prospective clients and help convert opportunities into new clients.

· Conduct in-home care consultations and work with our RN/nursing team to develop and maintain appropriate Plans of Care.

· Help coordinate successful new client starts, including caregiver matching, introductions and first-week follow-up.

· Work closely with scheduling to identify the right caregivers based on client needs, skills, personality, availability and location.

· Regularly check in with clients and families, proactively identify changing needs and resolve service concerns.

· Maintain accurate client information, documentation and activity in WellSky , our home care management software.

· Review caregiver notes in Wellsky and other client information to identify quality, safety or service issues requiring follow-up.

· Build strong relationships with clients and families and serve as a primary point of contact regarding their care and service.

· Help coach, support and develop caregivers assigned to your clients.

· Coordinate communication with physicians, home health, hospice and other healthcare providers when appropriate.

· Assist with quality assurance activities, including incidents, falls, complaints, infections and other client-care concerns.

· Develop relationships with selected referral and community partners and participate periodically in community outreach.

· Participate in an assigned after-hours/on-call rotation with other members of the management team.

What We’re Looking For

We are looking for someone who combines compassion with accountability . The right person communicates well, follows through, stays calm when problems arise and genuinely enjoys working with seniors, families and caregivers.

Preferred qualifications include:

· CNA/HHA certification strongly preferred and practical understanding of personal care and caregiving necessary.

· Experience in home care, home health, senior care, healthcare, medical offices or another service-intensive environment required.

· Associate or bachelor’s degree in nursing, social work, psychology, business, education or a related field preferred.

· Strong customer-service and relationship-management skills.

· Excellent verbal, telephone and written communication skills.

· Strong organizational skills and attention to detail.

· Ability to manage multiple clients, priorities and time-sensitive situations simultaneously.

· Comfort using technology, including Microsoft Office/Teams; experience with WellSky, ClearCare or similar healthcare/home-care scheduling software is a plus.

· Valid driver’s license, reliable transportation and automobile insurance.
