We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. This opportunity is open for a highly motivated and positive‑minded individual. Your Role in our Company Will Be: As an RN, you travel to a patient's home to administer services and help patients maintain their independence. Your duties include administering at‑home IVs, changing dressings, cleaning wounds, conducting in‑depth assessments, overseeing patient treatment plans, and updating doctors and the agency about patient health. Documentation and communication with the agency is done through Synergy Software. Availability Our patients need care 7 days a week, though you have flexibility in your scheduling. We ask that you be able to work at least two‑three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer). Work area: city close to you, with a group of patients who need care for that day. Responsibilities Provide nurse visits to home health patients (home visits). Perform an initial, comprehensive assessment. Document everything & upload to agency portal. Obtain and document physician orders. In consultation with the assigned Clinical Team Manager, initiate and regularly re‑evaluate and revise the plan of care. Coordinate care with the assigned Clinical Team Manager, physician, and other members of the home care team, informing them of significant changes in the patient's condition and needs. Perform re‑certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Complete and submit all related documentation. Assess the need for the services of other team members (PT, OT, ST, MSW, and HHA). Perform Home Health Aide supervisory visits per regulatory requirements. Demonstrate knowledge of regulatory requirements.
