The Lead Case Manager plays a critical role in supervising case management operations, guiding staff, and ensuring the delivery of high-quality care and services to clients. Position Summary The Lead Case Manager oversees all aspects of case management, including supervising case managers, coordinating complex cases, assessing client needs, determining risk categories, making referrals, implementing care plans, and planning follow-up services. This role also involves leadership in team development, compliance monitoring, and collaboration with internal and external stakeholders. Position Qualifications Bachelor's degree in Social Work, Psychology, Counseling, or a related field (Master's degree preferred). Minimum of three years of experience in substance abuse care and case management. Candidates without a relevant degree must have at least five years of direct experience in substance abuse care and case management. Demonstrated leadership or supervisory experience in a clinical or case management setting. In-depth knowledge of medical and psychosocial issues associated with substance use disorders. Exceptional skills in communication, documentation, and organization. Position Duties Supervise and mentor case managers, ensuring adherence to care standards and documentation protocols. Accept newly‑assigned cases after diagnostic intake and assess patient strengths/weaknesses to develop individualized case management plans. Facilitate team meetings and case reviews to monitor progress and address challenges. Maintain relationships with community agencies for wraparound services and referrals. Monitor and evaluate each patient’s progression and assist in obtaining necessary resources. Provide ongoing face‑to‑face reviews with patients to ensure continuum of care. Ensure plans are updated with accurate documentation of service objectives, outcomes, and corrective actions. Prepare case management reports for management and administrators. Actively participate in program‑related meetings and trainings to enhance service continuity. Collaborate with staff, community agencies, and family members to support patient goals. Uphold organizational values and use evidence‑based practices in service delivery. Participate in the review of clinical issues, policies, and procedures. Attend at least 2 hours of Clinical Oversight monthly. Ensure patient rights and privacy are protected in accordance with organizational policies, state law, and federal regulations.
