Weekend Admission Nurse (RN) (Full Time & Part Time Available)

Part time Registered Nurse Case Manager Anticipated Territory: Solano County Area Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day) Also willing to consider Full Time: Saturday and Sunday plus 3 week days. What you’ll do Conduct Start of Care (SOC) Admissions - OASIS Conduct follow ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols. Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
