DEPARTMENT: Vanderburgh County Health Department – Outreach/Education SUPERVISOR: Director of Outreach Services Planning and Implementing Activities/Program Support: Assists in planning, organizing, and coordinating resources necessary to operate and maintain the Pre to 3 Program. Implements program activities and develops daily work plans. Monitors initiatives critical to achieving measurable outcomes for eliminating health disparities. Staff Supervision and Coaching: Supports day-to-day staff supervision by helping mobilize teams to carry out program goals. Mentors, coaches, and assists with training and performance management of direct reports. Promotes professional development, a strength-based approach to client care, and strict adherence to safety protocols. Strategic Planning and Alignment: Contributes to strategic planning by analyzing surveillance data, best practices, and community input. Helps implement effective programming in response to maternal and infant outcomes. Coordinates the integration of office initiatives with community engagement activities across other divisions and stakeholders. Communications and Outreach Coordination: Coordinates the development and dissemination of materials used to inform families, providers, and the general public about vulnerable maternal and infant population needs. Manages outreach engagement schedules and assigns appropriate staff to participate. Program Quality and Compliance Assurance: Reviews and interprets state statutes, regulations, and guidelines on an annual basis or as needed. Ensures daily program operations meet the established compliance needs of the Pre to 3 program and reports findings to the Supervisor. Quality Improvement Execution: Implements and monitors quality improvement efforts to ensure increased program quality and efficient resource use. Participates in program financial, activity, and performance audits. Operates within departmental policies and provides feedback on Quality and Performance Management initiatives. Policy Support: Collaboratively contributes to the design of policy initiatives that address health equity and racial justice concerns while reducing the impact of health disparities. Program Evaluation and Recommendations: Evaluates daily program efficiencies and efficacy to identify improvement opportunities. Formulates operational solutions and recommends enhancements, revisions, or expansions of Pre to 3 initiatives, policies, and guidelines to leadership. Community Partner Collaboration: Cultivates and maintains strategic relationships with community partners, leaders, and organizations to improve maternal and infant outcomes. Facilitates the coordination of community efforts with health systems and collaborates internally to ensure seamless program operations. Writing and Reporting: Drafts and collaborates with staff on written reports, assessments, and program documentation. Maintains ongoing communication with the Supervisor and VCHD leadership. Communicates complex information clearly and concisely to professional partners, clients, and community members. Facilitates internal and external meetings and serves on advisory groups or committees. Ethics and Compliance: Monitors and ensures staff adherence to principles of ethical public health practice. Warrants daily compliance with all external requirements, mandates, rules, and regulations. Advocacy and Liaison Support: Advances the Health Department’s public health policy agenda by building relationships with local stakeholders. Maintains a collaborative working relationship with state agencies providing technical assistance. Interdisciplinary Professionalism: Fosters strong interdisciplinary relationships and a supportive clinical environment through professional behavior. Actively listens to peers, seeks input from other disciplines for problem-solving, and recognizes the accomplishments and contributions of co-workers to improve patient care. Operational Accountability: Fosters trusting relationships between patients, nurses, and team members to protect resources and promote agency reputation. Utilizes critical thinking to develop action plans for client care.
