Job Summary:

Lake Placid Health and Rehabilitation Center, a trusted member of the AOM/Eagle Point Healthcare family, is seeking a compassionate and experienced Social Services Director to join our team in Lake Placid, Florida. As a key member of our leadership team, you will oversee the social services department, ensuring our nursing home's commitment to individualized care and quality of life for our residents. Responsibilities:

• Oversee the social services department, including case management, social work, and recreational therapy services

• Develop, implement, and manage programs to meet the social, emotional, and spiritual needs of residents

• Collaborate with the interdisciplinary team to develop and maintain comprehensive care plans

• Recruit, train, and supervise social services staff to ensure exceptional service delivery

• Conduct assessments and support groups for residents and their families

• Develop and maintain relationships with community organizations and healthcare providers

• Participate in quality improvement initiatives to enhance the overall resident experience

• Ensure compliance with regulatory requirements and industry standards

Requirements:

• Bachelor's degree in Social Work, Counseling, or a related field (or equivalent experience)

• Minimum 1-2 years of experience in social services, preferably in a long-term care setting

• Strong leadership, communication, and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize tasks

• Familiarity with Florida's Long-Term Care Ombudsman program and elder law
